Family Medicine, Long Term Care

Covenant Health is seeking Family Physicians with an interest in Long-Term Care (LTC) to join the medical team at Youville Home Continuing Care Community in St. Albert. This opportunity is well suited for physicians who maintain an existing community-based practice and wish to expand their scope into LTC, as well as physicians who prefer to focus their practice exclusively within the Long-Term Care setting.

Physicians will have the opportunity to provide comprehensive, relationship-centred care to residents while enjoying continuity of care, a stable patient panel, and the satisfaction of making a meaningful impact on the lives of seniors and their families.

Whether integrated with an existing community practice or pursued as a dedicated Long-Term Care role, this opportunity provides physicians with the ability to deliver meaningful resident-centred care in a stable and rewarding practice environment.

Responsibilities

·         Provide longitudinal medical care to an assigned panel of LTC residents

·         Conduct regular assessments, medication reviews, and care planning rounds

·         Participate in family meetings and interdisciplinary case conferences

·         Ensure timely documentation in accordance with LTC standards

·         Provide on‑call or after-hours support within an agreed rotation (if applicable)
