Family Physician TEMPORARY & LOCUM – C3 Day Program
Locations: Carewest Beddington and Carewest Sarcee, Calgary, Alberta
Program Overview
The C3 Day Program provides coordinated, physician-led, interdisciplinary care to patients living at home who require ongoing medical oversight and day support services, including rehabilitation and recreational activities.
The program serves patients who would otherwise require placement in a long-term care (LTC) facility, supporting them in the community until admission to a care facility or end of life.
Patients are referred to the program by Home Care Case Managers, Family Physicians, and Specialists.
Role Overview
We are looking for physicians to fill one of two roles:

Participating Physicians serve as the Most Responsible Physician (MRP) for patients enrolled in the C3 Day Centre Program, providing ongoing medical management, clinical oversight, and medical sign-off.
Physicians work collaboratively with an interdisciplinary healthcare team at Carewest Beddington and/or Carewest Sarcee to assess patients, develop individualized care plans, and coordinate ongoing care.
Participating Physicians also share responsibility for after-hours medical coverage through a scheduled on-call rotation.

The on-call rotation provides coverage during the following periods:
- Monday to Thursday: 5:00 p.m. to 8:00 a.m. the following morning.
- Weekends: Continuous coverage from Friday through Monday at 8:00 a.m.
For patients admitted to Sarcee treatment beds, medical coverage is maintained 24 hours a day, seven days a week, through regular daytime physician services and the shared on-call rotation.
