• In conjunction with the Director of Integrated Home Care, the strategic plan for Home Care is implemented.
• Participate in the development of all aspects of Best Practices in Home Care.
• Acts as a key medical resource to the Home Care Team from the individual coordinator to the Director level.
• Perform a critical liaison with the medical community in all directions, including other Medical Directors, Primary Care leadership and front-line physicians, both urban and rural.
• Participate in complaint management.
• Attend committee meetings including Home Care Leadership, Seniors and Continuing Care Leadership meetings, Home Care Ethics Committee and the Quality Safety Committee.
• Assist the Seniors and Continuing Care leadership team and Corridor Clinical Department Heads with established utilization and quality improvement targets for Home Care services.
• Undertake to address barriers to system effectiveness and achievement of goals of Home Care.
• In collaboration with the Director of Integrated Home Care and the Seniors and Continuing Care Leadership team works in undertaking integration strategies, identifying trends, issues and development likely to affect work and outcomes within Home Care.
• Establish and update policies and procedures to facilitate the efficient effective operation of Home Care.
• Participate in the review of ethical issues in Home Care.
The successful candidate will hold a CCFP or FRCPC designation and be eligible for a Medical Staff appointment with Alberta Health Services. Clinical and leadership experience managing the care of seniors across home, community and continuing care spectrum will be beneficial.
The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
