Assisted Living Alberta is committed to providing services based on the population needs of Assisted Living and promoting a shared responsibility for improved health. 
 
The Contractor shall provide Services to Calgary in Advance Care Planning & Goals of Care Designations Support.
 

This position is responsible for providing input and leadership in the planning and coordination of the Advance Care Planning & Goals of Care Program within ALA. In partnership with the Coordinator, the Medical Leader will participate in the ongoing integration of Advance Care Planning & Goals of Care Designations programming in Calgary corridor including consultation, planning, education and continuous quality improvement
 

1.    Consultation
•    The Medical Leader may provide non-urgent support to palliative care consultants on clinical service when there is an escalation in a dispute regarding goals of care that exceed the clinical team's scope and require involvement of the hospital administrative team.
 
2.    Program planning
•    The Medical Leader will take an active role in planning program services and direction, providing input and advice from a physician perspective.
•    In partnership with the Coordinator and the Team, the Medical Leader will assist in the development and periodic updates to the Program Framework and Business Plan to guide the work of the program.
•    In partnership with the Coordinator and the Team, the Medical Leader will support stakeholder engagement within and outside of ALA to promote the adoption of a standardized approach to ACP/GCD and the integration into clinical workflows

3.    Clinician Engagement
•    The Medical Leader will engage physicians, physician/nurse practitioner groups and medical organizations to support adoption of ACP/GCD best practices in clinical setting, as well as engaging clinical champions to carry the program's vision forward

4.    Quality Improvement
•    The Medical Leader will participate in the further development of clinical practice guidelines, continuing medical education initiatives and other forms of quality improvement for medical services, including Level of Care documents and end of life policies.
•    In partnership with the coordinator, the physician will continue to support the implementation of the program to improve end of life decision-making and the documentation of treatment choices for patients in Calgary.

5.    Education
In collaboration with the ACP/GCD Team and Leaders in the Faculties of Medicine and Nursing, the Medical Leader will investigate best practices and develop a plan to provide education for physicians, nurses, residents and students on:
•    Decision-making at the end of life.
•    How to navigate cases in which there is a discrepancy between the medical team's opinion versus the patient's substitute decision-maker/ power of attorney in terms of the appropriate GCD, resulting in the need to engage in dispute resolution.
 
6.    Meetings
•    Monthly ACP/GCD team meeting
•    Bi-monthly Provincial ACP/GCD Community of Practice meeting
•    Quarterly meeting with the PEOLC Medical Director
 

The successful candidate will hold a CCFP or FRCPC designation and be eligible for a Medical Staff appointment with Alberta Health Services. Clinical and leadership experience managing the care of seniors across the continuing care spectrum will be beneficial.  
 
The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
Must read, write, and speak English.
