1 permanent locum position is available to exclusively to cover for the physician away that is available on Fanning 2W in the Rehabilitation and Community Transition Program (RCTP) starting October 1, 2026. This position will be reviewed annually.

Physician will be responsible for a panel of residents (roughly 10 individuals) on a post-acute unit, as assigned by the medical site lead. They will be designated as the attending physician for this group of clients.

The attending physician's role is to provide access to quality medical care through strong physician and interdisciplinary support.

They will be expected to work closely with site RNs, LPN, physiotherapists, occupational therapists, social worker, dietician, pharmacists as well as any other members of the interdisciplinary team. The clients are admitted from the ER or from an acute hospital unit.

We provide patient-centered specialized rehabilitation to individuals with impairments resulting from their acute medical and surgical condition.

A team of family physicians are part of the care team in the rehabilitation of these complex patients. The patients

will be medically stable prior to transfer and have potential for rehabilitation. There are also clients on active hemodialysis, admitted for rehabilitation managed by the RCTP MD’s.

-The Dialysis patient is the responsibility of the MD at RCTP 2 W Fanning when the patient is not on dialysis or not in the Dialysis unit.

Duties will include the following:

- Provide complete comprehensive medical assessment at the time of admission and through regular weekday visits according to a schedule agreed upon by the medical site lead and participating physicians.

- Complete medication reviews, review behavior maps and participate in Goals of Care conversations as required.

- Participate in as required interdisciplinary discharge planning conferences.

- Have experience using Connect Care. The physician assigned to a patient will do the admission and provide a timely and full discharge summary.

- Collaborate with the multidisciplinary team to attain the highest level of function possible to allow for discharge to home or to an appropriate supported level care.

- Collaborate with specialty teams providing consultation to sites such as Geriatric Mental Health

- Respond to concerns from nursing and other members of the care team during regular working hours. When present, the physician will care for additional residents identified by the interdisciplinary or participating physicians as required.

Time requirement:

- Work may include daily rounds Monday through Friday.  There will be weekend admissions and rounds during on call periods.

- Provide after-hour coverage (evenings and weekends) through participation in the home call on a rotating basis and weekend rounds (in person and home call).

When present at the facility, the physician will care for additional residents identified by the interdisciplinary or participating physicians.

 

 

·       Must have a family medicine designation with the College of Family physicians of Canada

·       Eligible for licensure and in good standing with the College of Physicians and Surgeons of Alberta and CMPA

·       Eligible for privileges within the Department of Family Medicine, Alberta Health Services

·       Clinical excellence in the described role, with previous inpatient experience and older adults with mental health issues being an asset.

·       Excellent clinical, interpersonal and communication skills

·       Demonstrated ability to collaborate in a team setting and to create patient focused solutions

·       Excellent interpersonal, communication and leadership skills

·       Desire to participate and contribute to quality improvement activities

·       It will be require attending an in-person interview and provide reference upon request
