Supporting Regional Facility/Zone access and patient flow:
• Physician representative available to Bed Capacity teams– either attends daily bed huddles or is available to bed huddle teams to assist with facilitation of physician communication.
Supports patient flow at a facility level to influence the regional patient flow.
• Promote understanding of the core services and community supports available at each site
• Liaise with RAPPID and rural sites to support admissions, transfers and repatriations as required
• Assist in changes to RAAPID call flow maps as needed to support effective flow (as above)
• Liaise with Continuing Care facilities to understand barriers to admission and options available
• Participates in Over Capacity Protocol management
• Participates in discussions with stakeholders and medical leaders regarding reductions of site services required for capacity reasons (surgical postponement, outbreak mgmt. etc.)
• Resource for medical leaders or MRP re operational challenges, assists in removing barriers
• Supports system issues management for complex patients
• Supports enhanced family experience
Consultations/Operational Leadership Support:
• Assist Zone Medical Affairs and Operations in recommending admissions and/or discharges in times of bed shortage, pandemic, disaster, strike, and other unforeseen circumstances.
• Familiarize with data/trends in site bed capacity over time (weekends, seasons etc.), to assist operations and align services with expected patient volumes.
• Assists with identification of patients appropriate for diversion to admission or transfer to rural sites
• Provide urgent consults to support home care teams, palliative teams and LTC sites regarding ER diversion
Virtual Rounding:
• Liaise with rural physicians and provide support with virtual rounding regarding complex medical patients.
ALOS Improvement Opportunities:
• Reviews data on ALOS – facilitates improvement options by communicating directly with Individual physicians. Mentor physicians in Know your Data project.
• Reviews all patients admitted > 14 days, supports care planning options
• Participates in care plan development for readmissions to Emergency
Liaise with Medical Leaders:
• Help develop infographics and learning materials for medical leaders and clinicians to clarify their role and help them to be effective in supporting patient flow
• Serve as a point of contact for medical or operational leaders/bed management staff in working through issues
Required Qualifications: The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
