Provide input into strategic planning for the Supportive Living and Continuing Care portfolio as well as the Seniors Health and Palliative Care programs within Calgary in order to ensure high quality patient and family centered care and system sustainability;
• Provide leadership, mentorship and support to physicians working within those program areas;
• Support development and implementation of policies and procedures that promote and support "best practice";
• Support quality assurance and quality improvement activities;
• Assist operational and medical leaders in the investigation and response to patient concerns.
• Recruitment, oversight and support of medical leaders in SPCC portfolio
Specific Committees/roles:
1. Co-chair, monthly Integrated Seniors and Community Care Leadership committee
2. Attend biweekly provincial ALA senior medical leadership meetings
3. Attend quarterly provincial meeting of SOOs and medical leaders
4. Monthly meetings with dyads from Palliative Care, Home Care, ISFL and Specialized Geriatric Services.
5. Annual Leadership performance reviews of medical directors under this portfolio.
6. Attend annual SPCC retreats.
Qualifications
The successful candidate will hold a CCFP or FRCPC designation and be eligible for a Medical Staff appointment with Alberta Health Services. Clinical and leadership experience managing the care of seniors across home, community and continuing care spectrum will be beneficial.
The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
